An eyeglass prescription tells an optical laboratory how to focus light and, when needed, align images. Sphere describes the main focusing power, cylinder and axis describe astigmatism correction, add provides extra near power, and prism shifts image position. The right and left eye are written separately. These entries do not measure eye health, visual-field loss, or how well the brain uses each eye, so a prescription should not be used as a substitute for an examination.
The plus or minus sign belongs to the number. Losing that sign changes the correction.
Start by identifying the eye and the distance
Forms may use OD for the right eye and OS for the left eye, or simply write right and left. OU refers to both eyes. Confirm the convention on the document before copying anything.
A distance prescription may be separate from a reading prescription. Progressive or bifocal orders combine distance power with a near addition. Computer glasses can use an intermediate target that differs from both.
Do not assume the first row is right on every website or order form. Match the printed labels.
Sphere describes the basic focusing direction
Sphere is abbreviated SPH. A minus sphere commonly corrects myopia, where distance vision is blurred. A plus sphere commonly corrects hyperopia and may also support near focus depending on age and purpose.
The magnitude describes lens power in diopters, not the percentage of vision lost. A person with a strong prescription can see very well with correction, while someone with a mild prescription can have poor vision from retinal or optic-nerve disease.
Plano, PL, or a zero entry means no spherical power in that part of the prescription. It does not mean the eye has no astigmatism or other problem.
Cylinder and axis work as a pair
Cylinder, abbreviated CYL, describes the amount of astigmatism correction. Axis describes its orientation and is meaningful only when cylinder power is present.
Axis is a direction, not an additional strength. A large change in axis can matter when cylinder is high, while axis can appear unstable when cylinder is very small.
Prescriptions can be written in plus-cylinder or minus-cylinder form. The same optical correction can be transposed between forms, which changes the written sphere, cylinder sign, and axis while preserving the lens effect. Apparent differences between two documents may therefore reflect notation rather than a real change.
The field decoder gives each entry one job
Use this decoder rather than judging the entire prescription by its largest number.
- SPH. Main focusing power for myopia or hyperopia.
- CYL. Amount of astigmatism correction.
- Axis. Orientation of the cylinder correction.
- Add. Additional plus power for near or intermediate work.
- Prism and base. Image displacement and its direction for an alignment need.
- PD. Pupillary distance used to position the lenses in the frame.
Some forms include segment height, optical-center height, or monocular pupillary distances. These are dispensing measurements rather than refractive diagnoses.
Add power does not replace the distance prescription
The near addition is combined with the distance correction in a bifocal, trifocal, or progressive design. It can also be used to calculate a dedicated near pair.
Add usually appears as a plus value because it reduces focusing demand at close range. The chosen amount depends on age, working distance, visual needs, and eye conditions.
Do not order readers by copying the add alone when significant distance sphere, cylinder, or prism is present. The final near power can require all components.
Prism needs both amount and base direction
Prism shifts the image to help the eyes align it. The prescription identifies prism amount and a base direction such as in, out, up, or down.
Prism does not simply strengthen an eye muscle. It can reduce double vision for some alignment problems, but tolerance and benefit should be tested.
Vertical prism may be split between the lenses in a way that looks different across prescriptions while producing the intended combined effect. Ask the prescriber or optician before deciding that one document is wrong.
Pupillary distance affects where the optics sit
Pupillary distance positions the optical centers relative to the eyes. A binocular value gives the total distance, while monocular values measure from the center of the bridge to each pupil.
Progressive lenses also require vertical fitting measurements taken with the chosen frame positioned correctly. A phone app estimate may not match a professional measurement, especially with asymmetry or a complex prescription.
The prescription and dispensing measurements are related but not identical. United States rules on releasing them can differ, so ask the practice which values are part of the prescription and which are recorded during frame fitting.
What changes are worth asking about?
Compare like with like. Make sure both prescriptions use the same cylinder notation and were written for the same distance. Then look at changes in sphere, cylinder, axis, add, and prism for each eye.
A change can reflect normal refractive development, cataract, blood-sugar fluctuation, corneal change, surgery, or measurement conditions. Numbers alone cannot identify the cause.
Sudden vision change should be examined rather than solved by ordering stronger glasses. Flashes, many floaters, a curtain, eye pain, or neurologic symptoms are not prescription-shopping problems.
Small written differences can arise from rounding, test responses, dry eye, pupil size, or different examination methods. Comfort with the trial lenses and the intended task helps the prescriber decide whether a change is meaningful.
Children and people with diabetes can show larger fluctuations when focusing effort or blood glucose changes. The timing and medical context belong beside the numbers.
Use the document to confirm an order
Before purchase, verify name, date, right and left eye, signs, decimal points, lens purpose, and prescriber details. Ask whether the prescription is current enough for the intended use.
For high power, prism, large eye-to-eye differences, or specialized occupational lenses, review when a complex glasses prescription needs closer attention.
The safest way to read an eyeglass prescription is field by field. Each abbreviation has a limited optical job. Understanding those jobs prevents copying errors while preserving the essential boundary between a lens recipe and an eye-health diagnosis.
